• Participants 13 & older: Instructed by Daihyo Masanaga Nakamura

• 12 and under: Instructed by Shihan Tats Nakamura

• Participation Fee: US$30/person

• Spectating and photo/video taking are prohibited

• Submit by September 30 for your team (No Exception)

• The following branch representatives are requested to have their respective Head Branch Chiefs complete this form on their behalf (titles such as Sensei and Shihan have been omitted): Balakrishnan Kuppusamy, Thanos Tziras, Dalamagas Georgios, Konstantin Nikonorov, Siyabonga Tyandela, Bonga Kubukeli, and Eric Snyders

• Branch representatives are required to wear a Dogi and participate in training if they wish to enter the seminar venue. Those who choose not to do so will be denied entry or asked to leave.

Agreement and Waiver

I am registering my Members for this Seminar as a representative of the members and/or their legal guardians.

I declare that:

• The Members will fully comply with and respect the rules and etiquette of I.K.O.N. during this event.

• The Members and his or her legal guardian understand there are risks involved in any physical activity, and therefore agree to not hold any sponsors, organizations or individuals responsible should an injury of any nature occur while taking part in this Seminar.  

• I, as an instructor of my members, take full responsibility to look after them during their stay in Taiwan, and to obtain travel insurance for any emergency including an injury.

• To the best of my knowledge, the Members does not carry virus such as HIV or Covid-19 which could infect others.
