The following branch representatives are requested to have their respective Head Branch Chiefs complete this form on their behalf (titles such as Sensei and Shihan have been omitted): Thanos Tziras, Dalamagas Georgios, Konstantin Nikonorov, Siyabonga Tyandela, and Bonga Kubukeli
Branch representatives are required to wear a Dogi and participate in training if they wish to enter the seminar venue. Those who choose not to do so will be denied entry or asked to leave.

Must possess a IKO Nakamura Membership Card and its number. If not, please order it before registering.
In regard to the gender selection, Honbu is aware of the issue related to transgender and gender non-conforming people. However, we will keep the traditional way until we reach a consensus among our Japanese and international representatives on the issue.
Agreement and Waiver
I am registering my Members for this Seminar as a representative of the members and/or their legal guardians.
I declare that:
The Members will fully comply with and respect the rules and etiquette of I.K.O.N. during this event.
The Members and his or her legal guardian understand there are risks involved in any physical activity, and therefore agree to not hold any sponsors, organizations or individuals responsible should an injury of any nature occur while taking part in this Seminar.
I, as an instructor of my members, take full responsibility to look after them during their stay in Taiwan, and to obtain travel insurance for any emergency including an injury.
To the best of my knowledge, the Members does not carry virus which could infect others.

The seminar fee will still be charged when the invoice is issued in October and November, even if this member is unable to participate.
